Puppy Agreement/Contract
Buyer: Please print, read, sign, and fax, email, or mail this contract back to me and I will copy and sign it and then send it back to you. This must be done by one week of transfer of ownership for the guarantee to be valid. [1] The following information is exclusively for the guarantee of your puppy form the time of purchase. A verification that the new owners/buyers have had a licensed Veterinarian with in 72 hours of receiving the puppy completed an evaluation of your puppy. Please supply a copy of the Veterinarian form to me and have a copy for your records. This must be completed to maintain the puppy’s health guarantee. Parvo is not covered by this contract, as the puppy would have only received 1-2 injections prior to going to their new home and family. Kennel Cough is also not covered as our puppies have only received 1 shot prior to leaving. A follow up shot is needed. This contract is for replacement puppies only if needed. [2] The purchase price of your puppy does not include shipping within the continental United States. [3] The deposit to place of your puppy is half of the purchase price unless otherwise stated. This deposit will be applied towards the purchase price of the puppy. [4] CANCELLATION If any reason you fail to pay for the balance due or decided not to take delivery of your puppy your deposit will be forfeited. [5] If the puppy is given to or transferred to another person(s), this guarantee is null and void. [6] If Lepto is injected into the puppy before 8 months of age the guarantee is null and void. [7] A puppy showing any signs of serious genetic on congenital defect will be replaced as follows: The puppy must show the defect by 1 year of age and be examined by a licensed Veterinarian. Breeder must be notified at once at the onset of the problem, in writing with a detailed report form your licensed Veterinarian, if the problem I not addressed as soon as possible, that will negate the contract as well. Breeders licensed Veterinarian will interpret the report, if he/she concurs, then a replacement puppy will take place. [8] A puppy with a defect must be returned with a specified problem or put down, with an autopsy to follow. All paper work involved with the said puppy must be returned, including original registration paper work. Any physical problem which is or could be the result of injury, over feeding, any other problems caused by human error does not constitute a replacement puppy. All necessary injections must be performed. Same applies to medical attention, if you fail to precede with medical help again the health agreement is null and void. [9] There are certain disorders that may be caused by environmental factors, i.e. Allergies, thyroid dysfunction, mange, other autoimmune disorders that are not covered by this health guarantee. Minor issues such as under bite, overbite, or a hernia are not covered. Pigmentation can take up to 1 year to fully come in, some times in the winter the nose may appear with a slight pink colors------ this is normal and then the summer sun will bring the pigment back. [10] Any and all Veterinarian charges incurred by the purchaser, are the sole responsibility of the purchaser and will under no circumstances be paid by the breeder or billed to the breeder, to include all spay and neuter charges. The breeder guarantees a healthy companion pet, but does not guarantee adult size, color and temperament and can not guarantee the puppy will breed. [11] This contract is null and void if the terms and conditions are not fulfilled by the purchaser/new parent[s]. [12] All Federal USDA and the State of Missouri Rules and Regulations govern all warranties and guarantees. All sales are considered initiated, transacted and finalized in Texas County, Missouri. All puppies will be examined by a state licensed Veterinarian prior to delivery. All expenses incurred with medical or in another set of circumstances are the responsibility of the buyer. Shipping expenses are never refunded, as it involves a third party I can not be held responsible for error or omission of an airline or the regulatory commission of the federal government. Missouri taxes will be added for any sales to a new owner that resides full time in Missouri. [13] If for any reason the puppy needs a new home, due to circumstances beyond your control you are no longer in a position to care for your puppy, I ask that you contact me immediately, with first right of refusal, before any other new home is considered. [14] A diagnosis of cocidiosis, giarrdia, parasites, or any other type of bacterial infection does not render your puppy unhealthy. These items can be found on the ground, in water and public places.
